Fill out the form below to join us on June 14th for Slip n’ Slide and Movie Night: 5pm to 10pm at the Church!

We are breaking out the Giant slip n slide for all the variations of water games: kickball, scatter ball, dodgeball, etc and we will end by setting up the projector and movie screen to watch LEGO Batman outside on lawn chairs and blankets! DINNER WILL BE PROVIDED!

Make sure to: Wear a swimsuit or clothes you can get wet, bring clothes to change into for afterwards, bring a lawn chair or a blanket, and most importantly BRING A FRIEND!

In the event of a medical emergency, I authorize the adult leaders and representatives of Bethany Baptist Church to seek medical treatment for my child. This includes, but is not limited to, first aid, medical care by licensed professionals, and transport via ambulance or emergency services if deemed necessary. I understand that every effort will be made to contact me immediately in the case of an emergency. I acknowledge that participation in the activity described above involves risk to the participant (and to the participant's parents or guardians, if the participant is a minor), and may result in various types of injury including, but not limited to, the following: sickness, bodily injury, death, emotional injury, personal injury, property damage, and financial damage. If a dispute over this agreement or any claim for damages arises, the participant (or parent/guardian) agrees to resolve the matter through a mutually acceptable alternative dispute resolution process. If the participant (or parent/guardian) and the activity sponsor cannot agree upon such a process, the dispute will be submitted to a three-member arbitration panel for resolution in accordance with the rules of the American Arbitration Association.
